Liability Waiver

Birthday Party/Event Waiver and Release Form

I,, on behalf of myself and my child,, hereby acknowledge and agree to the following terms and conditions in connection with the participation of my child in the Birthday party/Event hosted by La Posh Princess Spa Boutique ,LLC on

1. **Assumption of Risks:** I understand and acknowledge that participation in activities during the birthday party may involve inherent risks, including but not limited to minor injuries, accidents, or falls. I am aware that such risks are part of the activities and agree that my child’s participation is voluntary.

2. **Waiver and Release:** In consideration of being allowed to participate in the birthday party activities, I, on behalf of myself, my child, and our heirs, executors, and administrators, hereby release, waive, and discharge La Posh Princess Spa Boutique, LLC, its owners, employees, agents, and representatives from any and all claims, liabilities, or damages arising out of or in connection with my child’s participation in the birthday party/Event activities.

3. **Emergency Medical Care:** I understand that in the event of any medical emergency or injury to my child during the party, reasonable efforts will be made to contact me. However, in the event that I cannot be reached, I authorize La Posh Princess Spa Boutique,LLC to seek and consent to emergency medical treatment for my child as deemed necessary by medical professionals.

4. **Medical Information and Allergies:** I confirm that my child does not have any known medical conditions, allergies, or other health concerns that could be aggravated during the party activities. I agree to inform the host of any relevant medical information or allergies that they should be aware of.

5. **Supervision and Conduct:** I understand that I am responsible for supervising my child during the party and ensuring that they follow all safety guidelines and instructions provided by the host. I agree to maintain appropriate supervision of my child to ensure their safety and well-being or deem delegated child care provider with my child.

6. **Photography and Media Release:** I grant permission for photographs, videos, or other media recordings of my child taken during the birthday party to be used for promotional and marketing purposes by La Posh Princess Spa Boutique,LLC unless otherwise specified in writing.

7. **Personal Belongings:** I acknowledge that La Posh Princess Spa Boutique, LLC is not responsible for any lost, stolen, or damaged personal belongings during the party. I agree to take full responsibility for my child’s belongings and personal items.
By signing below, I acknowledge that I have read and understood the terms and conditions outlined in this waiver and release form and agree to be bound by them.
